Application Process Welcome to the first step of the Florence County CST process! Please fill out the Pre-Referral Checklist to determine the child’s eligibility. Once it is determined that the child may be eligible for CST, fill out the Referral Form with at least one parent/guardian. Each person that is involved in the referral process will need to provide their information on the Confidential Interagency Information Release Authorization. When completed, please drop off at Florence County Human Services (basement of the courthouse). If you have any questions while filling out the pre-referral checklist or referral form, feel free to call or email the CST Coordinator at (715) 528-3296 or fchsd@florencecountywi.gov If there is not an answer, please leave a message. After we receive the referral packet, the CST Coordinator will call to set up a date and time to meet with the family. During the first meet we will discuss the family's needs, what CST can do for them, and determine if it is a good fit. The individual/family and the CST Coordinator will then start to think about supports they would like/need on their team. The CST Coordinator will be helping throughout the whole process and will be able to answer any questions. Remember, no matter what, you are no longer alone!
