CHILDREN'S LONG TERM SUPPORT WAIVERS (CLTS):
This waiver is for children with developmental disabilities, physical disabilities and severe emotional disturbances. The purse of the waivers is to help families support their children with disabilities within their own home. The types of services depend on the child’s needs but could be anything from an adaptive aid, day services, respite or help with a home modification. Child must meet eligibility criteria and parents are assessed for their ability to pay.
BIRTH TO THREE:
Birth to Three (0-3) is a statewide program designed to assist families in meeting the special development needs of their infants and toddlers with developmental delays and/or disabilities. 0-3 is an interdisciplinary program whose goal is to bring the fields of education, health, and human services together to be of assistance to parents with children who have special needs. Eligibility requirements apply.
CHILDREN'S COMMUNITY OPTIONS PROGRAM (CCOP):
The purpose of CCOP is to provide a coordination approach to supporting families who have a child with a disability. This approach recognizes and maximizes the family’s capacity, resiliency and unique abilities with the intention of better support the in inclusion in all facets of community life for the child and family. We work with families to identify services and supports that help the family achieve outcomes.
COORDINATED SERVICES TEAMS (CST):
A team of family members, service providers, school representatives and community members working together to help families achieve individual and personal goals. Families involved have multiple service system needs. This program allows for a collaborative and cooperative partnership with families, children, friends, service providers and agencies to assist the child and family to have the support needed at home.
ELDER ABUSE/ADULT PROTECTIVE SERVICES:
The principal purpose of an elder abuse investigation is to determine and then offer to the elder person those services he or she might choose to prevent or relieve abuse. Every effort will be made by the lead elder abuse agency (Human Services) to offer services which will maintain the older person in his or her home. Among the services which may be offered are medical services, day or respite care, home-related services (meal deliveries, home repairs, etc.), transportation, dietary assistance, counseling, mental health services.
