The ADRC offers:
Information and Assistance. Staff is available by phone and by appointment in our office or your home. We listen to you and your unique situation, focusing on identifying needs, preferences, and support-both personal and financial. Staff will explore with and link you to programs and potential funding sources so that you can make decisions that are right for you.
The ADRC maintains a large resource directory that includes: in-home care; housekeeping and chore services; home modifications, safety, and maintenance; dementia and Alzheimer's resources; respite; transportation; senior nutrition / home-delivered meals; senior and low-income housing; assisted living, nursing homes and other long term care facilities; financial assistance through Social Security, SSI, Disability, Medicare, Medicaid and other benefit programs; Power-of-Attorney and guardianship information; adaptive equipment and more.

Long Term Care Options Counseling: staff provide an individualized consultation to help persons understand their choices when making decisions about where to live, what kind of help they need and where to find it. This process includes: Understanding your specific needs, goals and resources, financing long term care services, and eligibility for publicly funded or other programs. Long term care options counseling is free and confidential and can occur with you, family members and/or significant others. It is personalized and typically provided in a face-to-face meeting.
Do you need Health Insurance via the Affordable Care Act? A Certified Application Counselor is available through the Northern Health Center in Lakewood, and can help you successfully enroll in the Health Insurance Marketplace or BadgerCare. Call 715-276-6321, extension 232 to schedule an appointment in Florence County. There is no cost for the services of the Certified Application Counselor.
You may also visit The Health Insurance Marketplace at HealthCare.gov, or use the 24-hour-a-day consumer call center. The toll-free number to speak with a customer service representative is 1-800-318-2596, and hearing impaired callers using TTY/TDD technology can dial 1-855-889-4325 for assistance. The website can be accessed from desktop computers, smart-phones, and other mobile devices. In addition to English and Spanish, the call center provides assistance in more than 150 languages through an interpretation and translation service.
